Custom Fountain Pen to this extent?

I have been looking around at the custom fountain pen makers. A lot make pens with different materials, but I am wondering if there are any pen makers that do custom things with the clips and rings, etc. I am looking for something like the Montblanc Philharmonia (if you beat me on the one on ebay that ended recently I am very jealous).
